Application Checklist
APPLICATION DEADLINES:
- CURRENT CYCLE: November 1 (annually) new CIP trainee Applications for January 1 enrollment. Applications are now open!
- January 15 (annually) for Ministry of Health-CIP Funding Competition and new CIP trainee Applications for July 1 enrollment (new application forms coming soon!)
- May 1 (annually) new CIP trainee Applications for July 1 enrollment (new application forms coming soon!)
Please note: If the application deadline falls on a weekend, applications will be accepted until the following Monday.
New Trainee and MOH-CIP applications
Application Packages are to be emailed to the CIP Coordinator at uoft.cip@utoronto.ca
Please note: Your acceptance into CIP and/or for MOH-CIP funding is conditional upon acceptance into your graduate unit.
1. NEW APPLICANTS
- Application must be:
- initialed by applicant (clinical Department & Funding)
- signed by applicant
- signed by Program Director
- signed by prospective Research Supervisor
- signed by Graduate Unit (graduate stream), verifying acceptance, or supply confirmation of acceptance as available OR left blank if acceptance is still pending
- Documents to be submitted with the application are (Application Instructions):
- Application
- Program Director Support Form
- Research Supervisor Attestation Form
- Research Supervisor CV
- Letter of intent from applicant (one-page)
- A copy of the Graduate Program acceptance letter (when enrollment is confirmed)
- Current CV
- Canadian Common CV format required
- Research proposal (one-page)
- Most significant research-related contributions (one-page)
- Special Circumstances (optional, one-page)

For Postdoctoral Stream Applicants, in addition to the above:
- Start and end dates of the research project summary of previous research experience
- Copies of publications (first page only)
- Letter of reference from your previous research supervisor and/or Ph.D. supervisor
- Letter confirming to follow standard procedures for monitoring of progress in CIP, and listing any courses to be completed (if required)
- List of two additional faculty members who, together with your supervisor, will serve as your research advisory committee and meet with you every six months to evaluate your research and progress
2. CURRENT TRAINEES - Please note: trainees are not eligible to receive the MOH-CIP funding a second time if they were previously awarded the funding.

- Current CV (Canadian Common CV format required)
- Program Director Support Form
- Research Supervisor Attestation Form
- A one-page statement of current project, progress, and future plans.
